	Notes	Tube MUST be labeled with the patient's first and last name AND a second identifier (date of birth, SS#, or MRN) AND date and time of collection AND first initial and last name of the phlebotomist. Specimens will not be processed if all of the above requirements are not met. Positive antibodies will be identified, Antigen Types will be reported as needed. CPT codes 86870 and 86905 will be added as appropriate.
